Information collection
When you first attend an Osteopractice – London clinic, a thorough case history will be taken by your osteopath. Your case notes will be updated throughout the consultation, and during any subsequent consultations.
The purpose of your case notes is to provide you with optimal care. We may occasionally use your case notes for other purposes, such as to audit our processes and practices. Any information used in these circumstances would be entirely anonymised and would not identify individual patients.
If you would prefer to not have your information used in activities other than your personal care, please let us know.

You have a right to access your case notes. If you would like a copy, we can provide this for an administration fee.
